How to Register
Check you live within the boundary area
Enter your post code to check you live within the practice boundary area.
Complete a New Patient Registration Form
If you live within the boundary area, please complete our new patient registration. No proof of ID or proof of address is needed. Paper registration forms are available from reception, but registering online is recommended.
When you register, it's helpful to have your NHS number. You can use the NHS website to find your NHS number.
The practice operates a policy of non-discrimination on the grounds of race, gender, social class, age, religion, sexual orientation, appearance, disability or medical condition from patients wishing to join the practice.
Registering as an adult
If you are an adult, registering for yourself, the easiest way is to use the following link, which will take you to a special NHS website that will allow you to register with us.
Registering a child
You can register a child with the practice using the same link. But you have to remember that you are filling it in as if for them - and you are their carer. So:
- Remember, when the form asks for your name etc, fill in your child’s name.
- When the form asks if “you have a carer”, say yes and then give your own details as a parent or carer.
Your Named GP
All Patients registered with the surgery will have been allocated a named accountable GP who will be responsible for your overall care. However, you can book appointments with any of the GPs at the surgery. If you have a preference to which GP you are allocated, the practice will make reasonable efforts to accommodate your requests. If you wish to know which GP has been allocated to you, please ask reception at your next visit.
